Loyal viewers who have been watching dermatologist Sandra Lee, MD, aka Dr. Pimple Popper , for years probably think they've seen and know everything. When she posts a video of a bump protruding out of her patient's head you're quick to diagnose a cyst.

Not everything is a cyst, though. In Dr. Pimple Popper 's latest video, she extracts what everyone, even she, thinks is a cyst at first . However, she hints in the comments: "It's a little different from our usual cyst and blackheads". Definitely keep your eyes wide open for this one.

The video begins with a zoomed in view of a mans eyebrow. You, and Dr. Pimple Popper , can clearly see the lump popping up in the hairs. (The doc has a fancy mani, too.) Then, the clip jumps ahead to the cyst already cut open . Dr. Pimple Popper is leading the charge with her scalpel and fingers. Lurking inside, you can see a dark, pearl-like thing.

Dr. Pimple Popper asks her patient: "Has it been slowly growing?" He responds and confirms that yes it has been growing. It looks like that stops right now.

The doctor squeezes a little, pokes at it with her tweezers, and gets the contents moving. There's not one, but two little pearls hiding inside. The doctor pokes a little and shares an update, theres a twin thats in there! She gives a little squeeze andboomout pops another boba, err, mystery ball.

Dr. Pimple Popper's official diagnosis post-extraction: "It looks like a boba." Her assistants and others in the room agree: "We all thought it at the time." The dermatologist still hasn't updated her fans on the full pathology, that's saved for her All Access subscribers only.
