A left calf problem kept Hafeez out of Saturday's second one-day international, which England won by four wickets with 15 balls remaining.
The opener will miss the remaining three ODIs and single Twenty20 international at Old Trafford on September 7, which concludes the tour.
Hafeez, who scored just 102 runs in the six Test innings against England and a disappointing 11 in the first ODI, will remain in England to undertake his rehabilitation.
Fast bowler Irfan assumes his spot having initially missed out on a place in the one-day squad.
