Khachanov came through in two hours and 39 minutes on Sunday, emerging victorious 6-7 (4-7) 7-6 (7-3) 6-3.
The Russian fired down 14 aces and dominated the second set tie-break having lost the first set in the same circumstances.
Fifth seed Ramos-Vinolas appeared set for victory when he broke for a 2-1 lead, but he immediately surrendered that advantage before Khachanov struck what proved to be the key blow in the eighth game.
Khachanov is the ninth first-time winner of the year and the 20-year-old is projected to move up to 55 in the world rankings from 101 with this win.
